OHS Orientation and Registration Dates

*Registration Packets, found on the SLPSB and OHS website main page, must be filled out for New Student Registration and 9th grade registration. Returning students (10, 11 and 12 grade) do not have to fill in the registration packet, they fill in a demographic card.

*All students will have to fill in and sign the Registration Signature pages, also found on the OHS website main page or above.

Required Documents to be brought by student/parent for registration:

New Students and Transfers- copy of last report card or transcript if you have it, birth certificate, student's social security card, copy of drop paperwork from previous school, proof of shots record, 2 proofs of residency in parent or guardian's name (utility bill, apartment lease or mortgage paperwork, etc.).

*Registration Fee is $50.00. It must be paid in cash or money order or via the online payment links below. We DO NOT take Cash App, Zelle, Venmo, Apple Pay, Google Pay or any other App payment platforms.

Even though you pay for the parking tags online, you must fill in the application and provide a copies of the student's driver license, vehicle registration(s), and proof of insurance. (Note: if student will be driving multiple vehicles please provide registration and proof of insurance of all vehicles the student will be driving.)
